- SUMMARY OF RESPONSIBILITY - Job Summary (general statement of purpose or objective of position)Baptist Health Home Health/Hospice RN Case Manager provides, manages and coordinates care and services for an assigned caseload of patients.
- MINIMUM REQUIREMENTS - Knowledge, skills, abilities, license, registration, certification, education, and experienceEducation required: Graduation from an approved program of Nursing.
Required: Current Arkansas RN License.
Required: Current Arkansas Driver's License.
Preferred: Minimum 1 year of nursing experience. Home Health/Hospice experience preferred.

Schedule: Part-time, Days WEO, 7a-7p Sat / Sun; w/option for Weekday hours PRN
